When received, your application will be date and time stamped and placed in the appropriate category for unit size and type. If your application is incomplete, it will be returned to you for additional information. You will be allowed an additional 10 days to provide this information. Failure to respond will result in your application being removed from consideration. If your application is accepted, but there is no unit currently available, your application will be placed on a waitlist.

When your application is near the top of the waitlist, you will be contacted to schedule a personal interview. You will be notified in writing if at any step in the process it is determined that you do not qualify. In such an event, you will be given an opportunity to challenge your rejection in an informal hearing.

ABOUT SPRUCE BANK FARM III

 

FACILITY DESCRIPTION

​

Spruce Bank Farm lll is a housing facility for elderly and handicapped persons. It is owned by Woodbury Interfaith Elderly Housing Corporation, a non-profit organization and is financed by the U.S. Department of Housing and Urban Development (HUD). As such, its operations are governed by the regulations of that agency. There are 12 one-bedroom units, two of which are handicap adapted. The average unit size is 680 sq. ft. Each apartment has its own private entrance, and is assigned a parking space for one car.

ELIGIBILITY

​

Applicant must have an annual income from all sources of not more than $35,950 for one person and $41,050 for two persons to qualify for admission. Annual income consists of the gross household income. All income and medical expense must be verified annually to remain in compliance with the Department of Housing and Urban Development. Once a completed application has been processed and an applicant is approved for residency, that applicant becomes eligible for the unit type specified in his or her application.

APPLICATION

​

Applications are available at the Spruce Bank Farm office at 823 Main Street Woodbury, CT and online at www.sprucebankfarmct.com. All application questions must be filled in. If a particular question is not applicable, the applicant(s) should fill in N/A. The following documentation is also required to be submitted along with the completed application form:

 

 

  • A copy of a full-size birth certificate for all persons who will be living in the apartment.

  • A copy of a Social Security Card for all adult persons who will be living in the apartment.

  • A copy of a valid Driver’s License or current state identification card for all adult persons who will be living in the apartment.

  • A copy of an Alien Card for all non-US citizens.

  • Copies of the last three rent receipts or a copy of the current lease.

  • Proof of Income (State Assistance, SSI, Social Security Benefit, Employment, Child Support, Retirement Benefits, and any other income).

  • Employment:  6 consecutive weekly or 3 consecutive bi-weekly paystubs.

  • Pension: 3 months statements.

  • Bank Statements.

  • Checking: Statements for 6 consecutive months

  • Savings: Most recent statement

  • Life insurance policies. (Type of Insurance, Cash Value, Loan, and Penalty if applicable)

  • Proof of all other assets.

  • Copies of Identification card issued by a medical insurance company or provider, including Medicare and Medicaid.

  • Copies of court records (real estate, tax notices, marriage and divorce, judgment, bankruptcies or notarized power attorney documents)

 

APPROVAL & RESIDENCY

​

Once a completed application has been processed and an applicant is approved for residency, that person becomes eligible for the unit size and type specified in his or her application. However, if the unit requested is not currently available, the applicant will be placed on a waitlist for such unit. Applicants on the waitlist are classified into three tiers according to their adjusted gross incomes: Moderate, Low, and Very low. When units become available, priority is given to applicants in the lower adjusted incomes tiers. Applicants will be required to pay a background check fee of $16 once all references and verifications are received, and the qualifying criteria have been met. If an applicant turns down a unit, the applicant will be notified in writing of removal from consideration.

WAITLIST MANAGEMENT

​

Completed applications will be date and time stamped when received and placed on the appropriate waitlist for unit size and type. Applicants will be notified in writing of their position on the waitlist. Applicants are responsible for keeping their contact and financial information up to date and for reporting changes in a timely manner. The management of Spruce Bank Farm III will update the waitlist annually by sending letters to applicants notifying them of their current waitlist position and requesting that they confirm their continued interest in Spruce Bank Farm lll housing. Failure to respond to these letters will result in the applicant's name being removed from the waitlist.

 

 

Applicants confirming their continued interest in housing will be scheduled for an interview as soon as their name is near the top of the waitlist. Selection will be made on a first come, first served basis from the classification tiers in the following order:

 

Households with:    1. Extremely Low Income

                                 2. Very Low Income

                                 3. Low Income

Applicants will be given ten days to respond to a request for an interview. Failure to respond to a request for an interview will result in the applicant's name being removed from the waitlist. Whenever an applicant is removed from the wait list they will be advised that they will need to reapply and informed of their appeal rights.

RENT

​

The m monthly rent is calculated at 30% of the resident’s adjusted income from all sources. Adjusted income consists of the gross household income minus qualifying medical expenses if applicable. Utilities (heat and electricity) are included in the rent.

​

SERVICES     

 

There is an on-site Administrator and a Resident Superintendent on staff provided by DeMarco Management, the managing agent for the owner. The staff provides administrative and maintenance services for the facility and its residents. There is a community room and a maintenance building, and there are four coin-operated laundry facilities located at various locations throughout the facility. Trash receptacles are provided outside each apartment and are emptied by an outside service. Cable TV, telephone and internet services are not provided, but may be installed at the resident’s discretion. No meals, housekeeping, personal care services, transportation, pet care services, medical services or social worker services are provided.

PETS

​

Pets conforming the facility's pet policy are permitted.

SMOKING POLICY

​

Smoking is prohibited everywhere on the property.

AN ACT CONCERNING THE CONFIDENTIALITY OF SOCIAL SECURITY NUMBERS.

 

Be it enacted by the Senate and House of Representatives in General Assembly convened:

​

Section 1. (NEW) (Effective October 1, 2008) (a) Any person in possession of personal information of another person shall safeguard the data, computer files and documents containing the information from misuse by third parties, and shall destroy, erase or make unreadable such data, computer files and documents prior to disposal.

 

(b) Any person who collects Social Security numbers in the course of business shall create a privacy protection policy which shall be published or publicly displayed. For purposes of this subsection, "publicly displayed" includes, but is not limited to, posting on an Internet web page. Such policy shall: (1) Protect the confidentiality of Social Security numbers, (2) prohibit unlawful disclosure of Social Security numbers, and (3) limit access to Social Security numbers.

 

(c) As used in this section, "personal information" means information capable of being associated with a particular individual through one or more identifiers, including, but not limited to, a Social Security number, a driver's license number, a state identification card number, an account number, a credit or debit card number, a passport number, an alien registration number or a health insurance identification number, and does not include publicly available information that is lawfully made available to the general public from federal, state or local government records or widely distributed media.

 

(d) For persons who hold a license, registration or certificate issued by a state agency other than the Department of Consumer Protection, this section shall be enforceable only by such other state agency pursuant to such other state agency's existing statutory and regulatory authority.

 

(e) Any person or entity that violates the provisions of this section shall be subject to a civil penalty of five hundred dollars for each violation, provided such civil penalty shall not exceed five hundred thousand dollars for any single event. It shall not be a violation of this section if such violation was unintentional.

 

(f) The provisions of this section shall not apply to any agency or political subdivision of the state.

 

(g) Any civil penalties received pursuant to this section shall be deposited into the privacy protection guaranty and enforcement account established pursuant to section 19 of substitute senate bill 30 of the current session.

DESCRIPTION OF ADAPTATION FOR HANDICAP ADAPTED APARTMENTS

 

Spruce Bank Farm has 3 handicap-adapted units that have been adapted in the following ways to assist persons who have a physical or mobility impairment or handicap.  These are the adaptations that can be found in Spruce Bank Farm Handicap Adapted units.

 

  • The turnaround radius in the kitchen is 39”. The turnaround radius in the bathroom is 29”.  This is to allow sufficient maneuverable space for a wheel chair.

  • The sinks in the kitchen and bathroom have no cabinets or other installations  

      underneath that impede moving a wheelchair up to them closely.  They are
      approachable by adequate clear floor space.

  • Showers have a spray unit on a hose that can be fixed or hand-held.  There is a seat to which a person can transfer from a wheelchair or walker.  Grab bars have been installed in the bathroom next to the toilet and the shower/tub. 

  • The unit is located on an accessible route and is provided with a conveniently located and marked handicapped parking space.

  • Doors have sufficient space at either side to allow for maneuvering clearance for a person in a wheelchair.
